Many organizations require reporters to acknowledge certain observations, or procedural outcomes before an investigation is completed. By configuring predefined disclaimers, organizations can ensure consistent reporting practices across all reporters and investigations.
The feature is configured by Company Administrators when creating or editing questionnaires. Administrators can define one or multiple disclaimer options within the Wrap-up section of a questionnaire.
When reporters complete an investigation:
If only one disclaimer is configured, reporters will see the disclaimer text and can confirm it before completing the investigation.
If multiple disclaimers are configured, reporters will see a dropdown list containing all predefined options and must select the appropriate one.
When a disclaimer has been selected as mandatory, it is automatically included in the generated report.
If the questionnaire uses signatures, the disclaimer is displayed above the signature section, ensuring that it forms part of the official investigation record.
How to configure disclaimer options step by step
As an administrator go to Questionnaires tab:
Open the questionnaire by:
Navigating to the questionnaire you would like to configure and entering edit mode (blue button on the right).
Creating new questionnaire by clicking "Create questionnaire" blue button and entering edit mode.
Go to Wrap up section.
On the top of the Wrap up section you will find the Disclaimer setting.Mark the checkbox to make disclaimer selection mandatory.
Disclaimer will be visible to reporters only when this checkbox is enabled and they cannot finalize the investigation without confirming a disclaimer.
Configure disclaimer options by typing in the text field.
Use the "+" icon to add multiple disclaimer texts.
Use the "x" icon to remove disclaimer options.Save the questionnaire by clicking "Apply changes" blue button.
The disclaimer configuration is now active for all future investigations using this questionnaire.
