How to add a Questionnaire

  1. After logging in to your account and being on the Home Screen of the app, you will need to click the "Create questionnaire" button.

  2. On the next step – just add the questionnaire's title and click the Create button.

  3. After this you will be redirected to the questionnaire's overview screen. The Wrap Up section will be presented there by default. To start adding your sections and fields, click the Enter edit mode button:

4. To add a new section – click the respected button:

5. Set the Section title and click the Add section button:

6. At the next step you will be able to edit the Section's title, and add Steps of your Questionnaire:

7. While adding the specific Question to the Questionnaire, you will be able to select one of the three types:

  1. Text – a reporter will be able to enter text in the field as an answer to the given question.

  2. Multiple choice – if there are the particular options to choose between, this type of the question will be the best to set.

  3. Date – this one is the best to use if the answer involves entering a date.

When choosing the Multiple choice type, you will be also asked to set the minimum and maximum amount of the options a reporter may select:

If the question not necessarily needs to have one of the options selected, set the Minimum concurrently selected options to have a "0".
If the question may need to have at least one option selected, set the Minimum concurrently selected options to have a "1".
If the question is meant to have more than one option selected, set the Maximum concurrently selected options to have the number of options the given question has.

After you will add all the needed sections and the steps for it, click the Apply all changes button on the top.
